Rifampicin is a broad-spectrum antibiotic belonging to the rifamycin class, primarily used to treat various bacterial infections, notably tuberculosis and leprosy. It functions by inhibiting bacterial RNA synthesis, effectively eliminating susceptible bacteria.
Indications
Tuberculosis (TB): Rifampicin is a cornerstone in the treatment of active tuberculosis infections, typically used in combination with other antitubercular agents to prevent resistance development.
Leprosy: Employed as part of multi-drug therapy to treat Hansen's disease.
Prophylaxis for Neisseria meningitidis: Used to eliminate meningococcal bacteria from the nasopharynx in asymptomatic carriers, reducing the risk of meningitis outbreaks.
Dosage and Administration
Adults: The standard dose for tuberculosis is 10 mg/kg, up to a maximum of 600 mg daily, administered orally on an empty stomach, either 1 hour before or 2 hours after meals, to enhance absorption.
Children: Dosage is weight-based; typically, 10–20 mg/kg daily, not exceeding 600 mg per day.
Administration: Capsules should be swallowed whole with a full glass of water. Consistent daily dosing is crucial for treatment efficacy.
Precautions
Liver Function: Rifampicin can cause hepatotoxicity; therefore, liver function tests should be conducted before and during treatment, especially in patients with pre-existing liver conditions.
Drug Interactions: Rifampicin is a potent inducer of hepatic enzymes, which can reduce the effectiveness of other medications, including oral contraceptives, anticoagulants, and antiretrovirals.
Discoloration of Body Fluids: Patients should be informed that rifampicin may cause a red-orange discoloration of urine, sweat, saliva, and tears, which is harmless but can stain clothing and contact lenses.

Common side effects may include:
Gastrointestinal Disturbances: Nausea, vomiting, diarrhea, or abdominal pain.
Allergic Reactions: Rash, itching, or more severe hypersensitivity reactions.
