Intravenous Treatment for Acute Hypocalcemia and Cardioprotective Agent

Calcium Gluconate Injection 10% is a sterile solution used to treat acute symptomatic hypocalcemia and as a cardioprotective agent in hyperkalemia. Each 10mL ampoule contains 940mg of calcium gluconate, equivalent to 2.1mmol of calcium.

Indications

• Acute Symptomatic Hypocalcemia: Rapid correction of low serum calcium levels presenting with neuromuscular irritability or cardiac disturbances.
• Hyperkalemia: Provides cardioprotection by stabilizing myocardial membranes in the presence of elevated potassium levels.

Dosage and Administration

• Adults: The usual initial dose is 10mL of 10% calcium gluconate solution (equivalent to 2.1mmol of calcium) administered intravenously. If necessary, the dose may be repeated based on clinical condition and serum calcium levels.

• Pediatric Patients: Dosage and administration route depend on the degree of hypocalcemia and severity of symptoms. In severe cases, doses up to 2mL per kg body weight may be necessary. Subsequent doses should be adjusted according to serum calcium levels.

• Administration: Administer by slow intravenous injection or infusion to minimize risks of adverse reactions. Ensure the solution is clear and free from particulates before use.

Precautions

• Contraindications: Do not use in patients with hypercalcemia, severe renal impairment, or known hypersensitivity to calcium gluconate.
• Drug Interactions: Avoid concurrent administration with ceftriaxone in neonates due to the risk of precipitation. In older patients, ensure infusion lines are flushed thoroughly between infusions when using calcium-containing products.
• Monitoring: Regularly monitor serum calcium levels during treatment to avoid hypercalcemia. Observe for signs of extravasation during intravenous administration, as this may lead to tissue necrosis.

Possible Side Effects

• Common: Sensation of warmth, flushing, or a chalky taste following injection.
• Serious: Rapid administration may cause bradycardia, hypotension, or cardiac arrhythmias. Extravasation can lead to severe tissue injury. If serious side effects occur, discontinue use and seek medical attention immediately.
